Direct Deposit Authorization Form
Direct deposit is not mandatory; however, it is preferred. If you would like to enroll in direct deposit, please complete all the information below and upload a voided check or a direct deposit letter from your bank.

 I hereby authorize the company to directly deposit my pay to the account listed above. This authorization will remain in effect until modify or cancel it in writing.

HOUSING DEDUCTION & DESCRIPTION (please check all applicable sections)

I authorize a bi-weekly housing deduction to be taken directly from my payroll check. 

I also authorize a deduction for a security deposit, which will be used to cover any damages I may cause to the premises. If there are additional damages beyond the security deposit, I agree to the deduction of those costs, which will be communicated to me. 

The security deposit will be refunded if I return the keys and there is no damage to the premises.


Please complete the next two questions below only if your role is as a server or bartender!

 I authorize the deduction of 2.5% of my credit card tips to be deducted by my employer directly from my payroll check. This fee covers the expense incurred by the processing company and any administrative fees.

If my cash register is short of funds, I authorize my employer to deduct the amount from my paycheck. Deductions will not fall below minimum wage.
